    This form must be filled for each company separately. Products which will be tested must belong to same company.. 
    Test requests are taken in written and quotation is given in written by our laboratory.
    Test reports will be prepared according to the information on this form. Please fill the form completely.
    Trade mark, lot numbers, catalog numbers, size, etc.. have to be marked on specimens for starting biomechanical test

    Bu form herbir firma için ayrı doldurulmalıdır.  Test edilecek ürünler aynı firmaya ait olmalıdır.
    Test talepleri yazılı olarak alınır ve teklif yazılı olarak Laboratuvar tarafından verilir.
    Test raporları bu formda verilen bilgilere göre hazırlanacaktır. Lütfen formu tam olarak doldurunuz.
    Biyomekanik teste başlanılması için; ticari logo, lot numaraları, katalog numaraları, boyut vb. Bilgiler numunenin etiketinde belirtilmiş olmalıdır.
